The convenio especial in Spain
- The convenio especial is a public healthcare buy-in for residents who have no coverage through work, pensions or the S1 scheme.
- It is regulated by Royal Decree 576/2013 and managed by the regional health services.
- The core requirement is one year of continuous registration on the municipal padrón before applying.
- The monthly fee is a fixed amount by age band, lower for those under 65 and higher for those 65 and older.
- Pre-existing conditions are covered without exclusions, waiting periods or medical questionnaires.
- Prescription medicines are not subsidised, so users pay the full pharmacy price.
- Coverage applies within Spain and does not include a European health card for travel.
Who can benefit from the convenio especial?
Not every resident of Spain arrives with a route into the public health system.
Early retirees, non-working spouses and residents living on savings can find themselves outside both the contribution-based system and the UK pensioner arrangements.
For them, Spanish law provides the convenio especial, a public insurance agreement that provides access to the full health system for a fixed monthly fee.
This guide explains who can use the convenio especial, what it costs, what it covers and how to apply it in the Valencian Community.
What the convenio especial is
The convenio especial de asistencia sanitaria is a voluntary agreement between an individual and the regional health service.
It was created by Royal Decree 576/2013, published in the Official State Gazette, to guarantee access for residents excluded from ordinary coverage.
In exchange for the monthly fee, the subscriber receives the basic common portfolio of the national health system in the same conditions as any other user.
It should not be confused with the separate social security convenio especial, which allows people to keep paying pension contributions and has nothing to do with healthcare.
Spain’s national health system provides universal coverage to a population of more than 48 million according to INE figures, and the convenio especial exists to close the small residual gap.
Who the convenio especial is designed for
- Early retirees below state pension age with no S1 entitlement.
- Non-working residents living on savings or investment income.
- Spouses and partners without their own contribution record who are not covered as beneficiaries.
- Former workers whose entitlement lapsed and who have no other route back in.
- Long-stay residents whose private premiums have become unaffordable with age.
Working residents and autónomos do not need it, because contributions already cover them, and UK state pensioners are better served by the S1 route within the wider Spanish healthcare system.
Requirements: the one-year padrón rule
The central condition is one year of continuous, immediately prior registration on the padrón of a Spanish municipality.
Applicants must also be residents in the region where they apply and lack any other public coverage.
The padrón is therefore the clock that starts the countdown, which is one more reason to complete empadronamiento immediately after moving.
New arrivals on visas that demand health cover must bridge that first year privately, since visa rules require insurance from day one, as our guides to private health insurance for expats and the non-lucrative visa explain.
The standard fees are 60 euros per month for subscribers under 65 and 157 euros per month from age 65, fixed amounts that do not depend on medical history.
What the convenio especial covers and what it excludes
Subscribers receive the basic common services portfolio: primary care, specialists, diagnostics, hospitalisation and emergency care.
There are no medical questionnaires, no exclusions for pre-existing conditions and no waiting periods beyond the padrón year.
The significant exclusion is the pharmacy subsidy, so subscribers pay 100% of the price of prescribed medicines.
Coverage operates within the Spanish public network, and the scheme does not issue a European health card for travel abroad.

How to apply in the Valencian Community
The scheme is managed regionally, so Valencian residents apply to the conselleria responsible for health.
The application includes the padrón certificate proving the qualifying year, identification, and a declaration of having no other public coverage.
Once approved, the subscriber signs the agreement, sets up the monthly payment and receives the SIP card that opens every health centre in the region.
More than 12 million people in Spain also hold private health insurance according to sector data, and many convenio subscribers keep a reduced private policy for speed while relying on the public system for major treatment.
The convenio removes medical underwriting from the equation, which makes it particularly valuable for residents whose age or history makes private coverage expensive.
Our team reviews each client’s situation against the S1, private and convenio routes as part of the wider residency planning in Spain.
Common mistakes with the convenio especial
- Letting the padrón registration lapse and breaking the qualifying year.
- Applying before the year is complete and being rejected due to timing.
- Assuming medicines are subsidised and budgeting nothing for pharmacy costs.
- Cancelling private insurance before the convenio is formally approved.
- Confusing the healthcare convenio with the social security contribution convenio.
- Expecting coverage abroad, which the scheme does not provide.
